Insurance and Payment

We accept multiple insurance plans with the exception of Medicaid. We currently do not have a contract with the state and cannot accept Medicaid. We apologize for the inconvenience.
It is your responsibility to provide current insurance information at the time of your visit. It is the patient’s responsibility to become knowledgeable regarding your benefits-what it covers, does not cover, copayments, deductibles, testing, referrals, etc. Our responsibility is to submit a claim which reflects the services performed our office based on coding guidelines and regulations. We will bill both primary and secondary insurances.
Copays are due at the time of service. All charges not covered by insurance are ultimately the patient’s responsibility. If you need to set up a payment plan, please contact our office and we will be happy to assist you.
If you do not have insurance and plan to be self-pay, please advise our staff when scheduling your appointment. Payment will be due at the time of visit. Pricing quoted prior to a visit is an estimate and is subject to change depending on the services performed within the visit.
